5. The accident date and time identification including the hour in which the accident happened at morning or evening.
6. The formal authority which undertook with the accident and the name of the official who received the accident.
7. The necessity of finishing the private right and attaching the proof of that with the request.
8. If the prisoner who caused the traffic accident is included in the amnesty instructions, there is no need to submit any request for this matter.
